How to Create & Update your Patient Portal Account

New patients – use the SADIO app to:

  • Enter your medical history, medications, allergies, preferred pharmacy, AND
  • Sign consent forms prior to your visit.

You can use any device to enter information but you MUST use laptop or desktop to sign the consents.
